I collect nonpublic personal information about you only from:

• Information you provide to me
• Your history of doing business with me
• Government agencies whom I have contacted at your request

This information is considered private and will be disclosed to outside parties only:

• On your written request
• When required by law
• When necessary to adequately perform the services which I have contracted to provide

I maintain such records of past and present clients as may be necessary to provide you with present and future tax assistance. When such records are no longer useful, they will be shredded or otherwise destroyed, so as to preserve the privacy of such records.

I do my own mailing and persons not affiliated with Oak Park Business Services do not have access to any of your information. The law does not permit information obtained in the preparation of a tax return to be used for any other purpose.

This statement has been prepared for purposes of compliance with the Gramm-Leach-Bliley Act of 1999, regarding privacy of consumer financial information. Oak Park Business Services makes every effort to comply with all applicable laws, including Sec. 7216 of the Internal Revenue Code, regarding taxpayer privacy, and Treasury Department Circular 230, which governs the behavior of Enrolled Agents.
